Complex Malformation of the Inferior Vena Cava

Malformations of the inferior vena cava (IVC) are rarepresentations, exceptional in children, and are usually asymptomatic.They are caused by disturbances in the embryologicalformation of the venous system or can develop as a result ofperinatal venous thrombosis with secondary impairment of thevenous development. We report the case of a 14 year old boy,admitted for pediatric evaluation before undergoing plasticsurgery in order to remove superficial varicose veins of thelower abdomen. The patient presents with inequality incircumference and length of the legs. Laboratory investigationsare normal and the abdominal ultrasound describes hypoplasiaof the retrohepatic segment of the inferior vena cava. Thediagnosis of complex malformation of the abdominal deepvenous system (retrohepatic vena cava atresia, cavo-cavalanastomosis through azygos veins, abnormal formation of theinferior vena cava with the absence of the left iliac vein) wasestablished through a CT angiography. The presence ofabdominal varicose dilations should indicate the necessityto closely look for malformations of the portal and or cavalvenous systems.
